One of the most common questions I get asked is: how do I decide what joint supplement to use? Epitalis, Pentosan Polysulphate, Glucosamine, or Hyaluronic Acid?
Unfortunately, the answer isn’t a one-size-fits-all solution. Instead, I make recommendations to my clients based on factors such as the horse’s stage of life, discipline, and the presence of arthritic changes.
When it comes to joint management, a well-established multi-modal approach is ideal. This includes the use of systemic medications, a combination of joint treatments (either with steroids or biologics), and physical therapy (e.g., joint stability training and mobilization).
Both 4Cyte (Epitalis Forte) and Pentosan Polysulphate contain active ingredients that are purported to have disease-modifying properties, earning them the classification of DMOADs (disease-modifying osteoarthritic drugs).
Let’s take a deep dive into these two key products, exploring what they have to offer and how they affect the body. Hopefully, this will help you make an informed decision about which option is most suitable for your horse.
Epitalis (Trade Name: 4Cyte™)

What it is: An oil extract derived from the seeds of a plant (Biota orientalis), containing the active ingredient Epitalis. It is designed to support and repair joint cartilage.
How it works:
Reduces concentrations of prostaglandin E2, a potent hormone that mediates inflammation.
Stimulates cartilage synthesis and slows the breakdown of joint cartilage.
Administration: A daily oral gel (Epitalis Forte) or powder (which can be less palatable) added to feed.
Key Benefits:
Convenient for daily use.
Palatable and easy to administer.
Supports long-term joint health and repair.
Pentosan Polysulphate (Trade Names: Pentosan [CEVA] or Arthropen [Randlab])

What it is: A heparinoid compound derived from beechwood. This prescription medication is known for
its anti-inflammatory and cartilage-protective properties.
How it works:
Reduces inflammation within the joint.
Improves joint fluid quality and supports cartilage repair.
Stimulates the production of proteoglycans (e.g., chondroitin) and hyaluronic acid (a natural joint lubricant).
Administration: Intramuscular injections, starting with an initial course of one injection weekly for four weeks, followed by monthly injections as maintenance.
Key Benefits:
Reduces inflammation.
Improves joint lubrication.
Suitable for acute or ongoing conditions.
Both products are ideal for: Horses needing proactive, long-term joint support or showing early signs of stiffness or discomfort. They can also be used as a preventative measure before the onset of arthritic inflammation. Remember, every horse is unique and it is best to discuss these options with your veterinarian to tailor the best plan for your equine partner's needs.
Stay Proactive with Joint Health
Regular veterinary assessments and early intervention are key to maintaining your horse’s comfort and performance. Whether it’s incorporating 4Cyte into daily feeds or scheduling a Pentosan treatment plan, having a holistic management plan for your horse is key!
